North Central Oklahoma Continuum of Care Lead

Through the infrastructure of the Continuum of Care (C0C) including providers, individuals with lived experience, and subject matter experts, the CoC Board determines policy and makes decisions for providers and agencies connected to the Continuum of Care.  The Continuum of Care Board plans for the region, convenes diverse stakeholders that address and are impacted by homelessness, and makes regional policy recommendations to local leaders.  We stand ready to lean in, support, and provide expertise to local efforts addressing homelessness.

North Central, Southeast, and Northeast HMIS Lead

CDSA, Inc. is the lead agency and host site for the North Central, Southeast, and Northeast Oklahoma Continuum of Care Homeless Management Information System (HMIS).  HMIS utilizes Wellsky Community Services (formerly known as Sharepoint), a web based application, to capture client level information and coordinate case management of services for individuals experiencing homelessness or near homeless in the service area.

The HMIS system connects agencies who offer shelter, housing, and prevention services in eight counties-Creek, Garfield, Grant, Kay, Noble, Osage, Pawnee, and Payne County.  All Emergency Shelters and HUD funded programs are recording client data, case management and services provided in HMIS.  Some services include food, shelter, clothing, transportation, counseling, case management and bill payment assistance. 

HMIS is funded through a grant from the U.S. Department of Housing and Urban Development for Continuum of Care.
